|For your child to be enrolled for the FIRST WEEK of school - ALL PAPERWORK MUST BE RECEIVED NO LATER THAN AUGUST 15, PRIOR TO THE CURRENT SCHOOL YEAR. APPLICATIONS RECEIVED AFTER THIS DEADLINE WILL DELAY ENROLLMENT STATUS UNTIL THE SECOND WEEK OF SCHOOL IN SEPTEMBER.
It takes time to process paperwork, hire, train and maintain appropriate staff, and avoid over enrollment. Space is limited, enrollment is based on first come, first served basis. Please, plan ahead!
Obtain an application packet and complete the three forms attached: Tuition form, Emergency Contact/Parent Consent form, and Medical/Health form.

All registration forms must be accompanied by the Application Fee and the Enrollment Deposit equivalent to one month's tuition payment. Mail all forms for the Extended Day Program to the address printed on the top of the application.
Once your application is received, you will receive a letter confirming your child's enrollment status in the Extended Day Program. No child will be permitted to attend the program until ALL applicable forms have been submitted.
enrollment agreement will be mailed to you with your first invoice. This agreement must be signed and returned with your first payment to the Arizona address indicated on the bottom of the invoice.
